After you finish this report form, go to the TGA website and report your vaccine reaction on their online form. Anyone can make a vaccine report so you do not have to leave it to your doctor, but always ask that they report it first and if they refuse, report it yourself.
It is important to report all vaccine reactions so that they can stop dismissing them as rare, remember it is estimated that less than 10% of vaccine reactions are reported, this needs to change.
